The Nursing Home Transition Program (NHT) assists and empowers individuals who want to move from a nursing facility back to a home of their choice in the community. Through the NHT program, individuals along with their families and caregivers receive the guidance and support they need to make informed choices about their long-term services and supports. The program assists individuals through the process of moving out of an institution and eliminating barriers in service systems so that individuals can receive services in the setting of their choice.

How does it work?
An NHT Coordinator will meet with the individual requesting services in the nursing home to identify existing barriers that prevent them from returning home. The NHT Coordinator will then assist the individual in creating a plan to eliminate the barriers for a safe return to the community. Individuals who select Abilities in Motion as their NHT provider may receive assistance in setting up the following services:
Providing accessible home modification
Arranging for personal care services and other community-based services
Assistance locating affordable/accessible housing
One-time funding for security deposits and essential items for the home
