Community HealthChoices Managed Care

Community HealthChoices (CHC) is a mandatory managed care program for many Pennsylvanians, including individuals who receive Medicaid long-term services due to income and disability.  CHC’s major principals are in line with the goals of Abilities in Motion (AIM) – to allow people to receive services in their communities, teach them the skills needed to participate in the workforce, and experience an improved quality of life.

CHC is rolling out three managed care organizations who will assume the funding of your services – AmeriHealth Caritas Pennsylvania, PA Health & Wellness, and UPMC Community HealthChoices.  As CHC is rolled out in three waves across the state in 2018, 2019, and 2020, Pennsylvanians who receive both Medicare and Medicaid and residents who receive long-term services and supports due to physical disability will need to select one of these three providers.
