Consumer Directed Model

Abilities In Motion's Consumer Directed Model provides consumers with more control over their own personal care. This allows consumers, who are capable of directing their own care, to hire their own attendants. Attendants can be family members (limited restrictions apply), friends or trusted neighbors. Attendants will become employees of consumers and will submit approved timesheets to our office for processing. See Financial Management Services for more details. 

 In Home Support Visits - Abilities In Motion understands that the Consumer Directed Model carries much greater consumer responsibility. For this reason, we supply consumers with additional initial support, along with our long range supports. A Consumer Enrollment Agent will visit your residence and spend as much time as necessary to explain the program in detail. You will then be provided with in-home support visits approximately on the 45th day of service and then again at 90 days. These visits will allow a consumer to pose any questions they may have regarding the program. After 90 days the consumer can request additional in-home support visits when they feel necessary.

RN (Registered Nurse) Visits can be requested at any time by a consumer for training on various procedures. Consumers are encouraged to utilize this support service when they have questions regarding Health Maintenance Activities including transferring, bathing techniques, or any other request a consumer may have. RN training will be provided for additional supports with maintaining consumer independence.
  
Advertising can be requested at any time for recruitment of consumer directed attendants. AIM has two methods for placing an ad in a local newspaper or publication: (1) The consumer can write and place an ad in a local publication of their choice and submit a request for reimbursement to AIM, or; (2) Consumers can utilize a predetermined ad that consumers designed and AIM will place the ad on behalf of the consumer in their local area newspaper or publication..  
  
Abilities In Motion provides consumers with the highest pay scale in the industry!! We encourage consumers to compare the pay scale that AIM offers consumers against other providers who set attendant pay rates! AIM does not set pay rates; we supply the consumer with a pay range, the consumer chooses the rate they want to pay their attendant. AIM does not dictate the pay rate of your attendant! This pay scale is then utilized by consumers to determine how much to pay their attendant. Abilities In Motion does not set the pay rate, you, the consumer, set the pay rate. This allows the consumer ultimate control over their own care. This pay scale ranges from the State minimum wage to a pay scale cap amount that is based on funding.   

 Abilities In Motion also offers Consumer Directed Attendants the ability to participate in a Medical Supplement Plan that offers partial reimbursement or immediate discount for common medical requirements such as doctor appointments, prescriptions, surgery, etc. This plan is NOT a Major Medical plan, it is a medical discount plan. Reduced premiums are offered to consumer attendants via a statewide initiative to provide some health care assistance.  

Timesheets Vs. Call in system:
Abilities In Motion utilizes the very simple timesheet process rather than the call in system. Consumers or Attendants will not have to worry about forgetting to call in or be hassled with utilizing this system. Consumers and Attendants have always chosen to utilize timesheets rather than the call in system. Abilities In Motion believes in CHOICE, so when the consumer says they want timesheets, they get timesheets! Choose Abilities In Motion as your provider; consumer input dictates our procedures!

Home Health Agency Model

Abilities In Motion's Home Health Agency Model provides a choice to consumers to choose from various Home Health Care Agencies to provide their personal care. Abilities In Motion contracts with many local Home Health Agencies so that the consumer has the ultimate choice over who is providing their care.

Combination Model

Abilities In Motion is all about "Consumer Choice" and to provide choice, we welcome consumers to utilize a combination of both Home Health Agency Model AND Consumer Directed Model, this is called the "Combination Model"

This allows consumer the ultimate control over directing their own care. A consumer can choose to have an Agency Attendant provide a portion of their care and then a family member, friend or trusted neighbor can provide a portion also. Combination model works very well for setting up a back up plan. The consumer directed model encourages setting up multiple attendants so that there are attendants available for back up when a primary caregiver gets ill, goes on vacation, etc.
