AIM Service Coordination: At Aim Service Coordinators work with you and your family to design a service plan that is right for you. We listen, we provide information, we make sure you understand all the options and services available to you. Your Service Coordinator will work hard to ensure you receive the best possible care within the guidelines of the waiver.

Serving Berks, Bucks, Chester, Clinton, Dauphin, Lancaster, Lebanon, Lehigh, Montgomery, Philadelphia, Schuylkill counties.

CHOOSING HOME INSTEAD?

Want to remain living in your home, your community? Home and Community Based Services Waiver Programs help you do just that. Waiver funds can provide such things as in-home personal attendants, community integration services, personal emergency response systems, transportation, and in some cases, home modifications. All of these services contribute to achieve the goal of helping you remain as independent as possible and live the lifestyle of your choosing.

The Enrollment Experience

Waivers differ in terms of the population they serve, the number of people they can serve, the criteria that must be met in order to qualify, and the services provided. To apply for waiver services, consumer’s go through a qualification and enrollment process administered by Pennsylvania’s Independent Enrollment Broker. AIM can get you started in the process and “coach” you along the way. During this qualification and enrollment process, you will be asked to choose a Service Coordinating Agency. We hope you choose the excellence that AIM Service Coordination Provides.

Service My Way When you want Ultimate CONTROL!

A new service option may be available to you called Services My Way (SMW). With SMW, you work with your Service Coordinator to choose the goods and services you want, direct as many of those services as you want, and are the driving decision-maker in ways that can improve your quality of life. This way you manage your own flexible budget to design and purchase disability-related goods and services. Based on your plan budget, you develop your spending plan to purchase goods and services that will assist you in meeting your needs and goals as identified in your service plan.

OK, you qualified for services. Now it’s time to choose your personal attendants. AIM Service Coordinators can help you choose the model of service that best suits your needs.

Consumer Directed Model – have a trusted friend or family member that you would like to hire as your attendant? This model allows consumers, who are capable of directing their own care, to hire their own attendants. Attendants can be family members (limited restrictions apply), friends or trusted neighbors. Attendants will become employees of the consumer and will submit approved timesheets to our office for processing.

Home Health Agency Model – Would you rather have an attendant from a home care agency? Abilities In Motion works with many local Home Health Agencies so that the consumer has the ultimate choice over who is providing their care.

Combination Model - Want to have a little of both? You can do that too. A consumer can choose to have an Agency Attendant provide a portion of their care and a family member, friend or trusted neighbor provide a portion as well.
