How does Service Coordination work?
The Service Coordinator annually evaluates the Consumer's strengths, needs, preferences and desired outcomes. The Service Coordinator designs a service plan to meet the Consumer's goals which may include personal assistance service, durable medical equipment, home modifications and skilled services. The Service Coordinator monitors the goals throughout the year and assists the Consumer with problem solving and provider referrals.
How do you know if you are eligible?
Consumers are determined to be financially and clinically eligible by an independent enrollment broker and your local Area Agency on Aging.
Where is Service Coordination provided?
Service Coordination is provided in Berks, Lancaster, Lebanon, Chester, Montgomery, Lehigh, Delaware, Northampton, Philadelphia, Schuylkill, Dauphin and Bucks counties.
How are Service Coordinators assigned?
Service Coordinators are assigned to a Consumer by the geographical region of where each Service Coordinator works and lives.
