Nursing Home Transition

Abilities In Motion’s Nursing Home Transition Program provides intense support and assistance to nursing home residents that wish to leave a nursing facility and move back home to the community to live a life of independence.

We are available to provide assistance with securing and establishing housing, coordinating accessible home modifications, training for independent living and the providing of health and medical services through connections to home and community based services.  Funding may be available to help with one time expenses such as security deposits, essential furnishings and moving expenses.  We also offer peer support and community reintegration education. 

Abilities In Motion directs the Nursing Home Transition Program in Berks, Lancaster and Lebanon and Lehigh Counties.

 

Program Specifics

A candidate:

  • must be under the age of 60
  • must have the determination and means to live independently
  • must have monthly subsistence income/ funds

Service is:

  • for one-time cots to secure housing
  • available to provide home modifications before returning home.
