Responsibilities & Qualifications:

SERVICE COORDINATION

AIM is dedicated to advancing the rights of persons with disabilities in order to promote a full life in the community through the prevention, and elimination of physical, psychological, social and attitudinal barriers which serve to deny them the rights and privileges common to the general public.  Services are not limited by type of disability or age.

The Service Coordinator plays an important role in ensuring that participants receive the services and supports that meet their assessed needs.  The Service Coordinator works closely with and at the direction of participants whenever possible, to assess; identify needs; plan and coordinate services; and monitor quality of services.  The Service Coordinator monitors these identified services in order to support the participant in their independent living environment.

Ideal candidates will:

  • have a Bachelor’s Degree in social work or related social sciences;
  • reside in the region specific to the position;
  • preferably have at least three years of experience in community-based social work;
  • be able to work independently from remote sites with minimal supervision; and,
  • have a valid driver’s license and an insured vehicle.

 

NURSING HOME TRANSITION

The NHT Coordinator is responsible for overseeing the transition of consumers moving from nursing homes to a residential setting.  The NHT Coordinator will work collaboratively with numerous social service agencies, family members, nursing home staff, Ombudsmen and therapists to successfully plan and organize the transition.

Ideal candidates will:

  • have a Bachelor’s Degree in social work or related social sciences;
  • have at least two years of prior experience in social services; and,
  • have a valid driver’s license and an insured vehicle.

SERVICE COORDINATION DEPARTMENT

Intake Administrator for Long Term Services & Supports – Bilingual Spanish/English

Location: Berks Office

Position Summary: The Intake Administrator is a full time position to process and manage the intake/referral process of the LTSS department.

Essential Functions: 

  • Receive, screen and assign intakes for LTSS programs
  • Confirm eligibility for consumers entering LTSS programs
  • Document activity in required systems
  • Maintain tracking of all referrals and intakes
  • Prepare reports as requested
  • Schedule visits for service coordinators
  • Monitor compliance with policies and procedures
  • Answer and direct phone calls to the proper resource
  • Provide administrative support as needed
  • Bi-lingual in English and Spanish
  • Proficient with Microsoft Outlook, Excel and Word
  • Must have excellent customer service skills

Position Summary: The Service Coordinator (SC) is a position within the Home and Community- Based Services/Service Coordination department.  The SC position is funded through the Department of Human Services.  The Service Coordinator plays an important role in ensuring that the participants receive the services and support that meet their assessed needs.  The Service Coordinator works closely with the and at the direction of the participant, whenever possible, to assess, identify their needs, plan and coordinate their services and monitor quality of services including MA State Plan Services and other medical, social, and educational services.  The Service Coordinator monitors these identified services in order to support the participant in their independent living environment.

NURSING HOME TRANSITION DEPARTMENT

Nursing Home Transition Coordinators – English-Speaking

Location: York/Adams Counties
Location: Montgomery County
Location: Chester County

Position Summary: The Nursing Home Transition Coordinator (NHTC) is responsible for overseeing the transition of consumers moving from nursing homes to a residential setting. The NHT Coordinator will work collaboratively with numerous social service agencies, family members, nursing home staff, Ombudsmen and therapists to successfully plan and organize transition.  Candidates need a Bachelors’ Degree in Social Work or a related field. Preferred candiate should have three years in community-based services. Ability to work independently and build relationships with Social Workers in Nursing Homes.  Must have good communication skills.
