SERVICE COORDINATION DEPARTMENT

AIM is dedicated to advancing the rights of persons with disabilities in order to promote a full life in the community through the prevention, and elimination of physical, psychological, social and attitudinal barriers which serve to deny them the rights and privileges common to the general public.  Services are not limited by type of disability or age.

AIM is seeking candidates for 2 open Service Coordinator positions in the following regions:

  • York/Lancaster/Lebanon Counties

The Service Coordinator plays an important role in ensuring that participants receive the services and supports that meet their assessed needs.  The Service Coordinator works closely with and at the direction of participants whenever possible, to assess; identify needs; plan and coordinate services; and monitor quality of services.  The Service Coordinator monitors these identified services in order to support the participant in their independent living environment.

Ideal candidates will:

  • have a Bachelor’s Degree in social work or related social sciences;
  • reside in York, Lancaster or Lebanon County;
  • have at least two years of prior experience in social services;
  • be able to work independently from remote sites with minimal supervision; and,
  • have a valid driver’s license and an insured vehicle.

NURSING HOME TRANSITION DEPARTMENT

AIM is seeking a candidate for a Nursing Home Transition (“NHT”) Coordinator to provide coverage for York/Lancaster/Cumberland and Adams Counties. 

The NHT Coordinator is responsible for overseeing the transition of consumers moving from nursing homes to a residential setting.  The NHT Coordinator will work collaboratively with numerous social service agencies, family members, nursing home staff, Ombudsmen and therapists to successfully plan and organize the transition. 

Ideal candidates will:

  • have a Bachelor’s Degree in social work or related social sciences;
  • reside in either Lancaster or York county;
  • have at least two years of prior experience in social services; and,
  • have a valid driver’s license and an insured vehicle.
